These prices apply only for CCMS members and only at this Mortuary. They are guaranteed to be valid at the time after-death care is rendered only if prepaid. All service arrangements are to be made directly with the Mortuary, using the "Instructions" form provided by the Society. All payments for service should be made directly to the Mortuary. These prices will be in effect January 1, 2023 through December 31, 2023.
PLAN A: DIRECT CREMATION --------------------------------$1,265.00
Price includes removal of body from medical facility or home, delivery to crematory in a minimum cardboard container, cremation and minimum plastic urn suitable for mailing. Special Note: Cremated remains are to be picked up by survivors (or their designee) at the Mortuary unless shipping is arranged. Cremated remains may be held by the Mortuary for a maximum of 30 days.
PLAN B: DIRECT & GREEN BURIAL --------------------------------------$1,954.00
Price includes removal of body from medical facility or home and delivery to local cemetery in minimum, cloth-covered flat-top casket.
For plans A and B, arrangements for cemetery, mausoleum, columbarium or other related services, must be made separately with the providers of such services..
PLAN C: BURIAL FOR VETERAN OR SPOUSE----------------------------$2,348.00
Price includes removal of body to Mortuary for preparation, delivery to Riverside National Cemetery, Los Angeles National Cemetery, or National Cemetery in Bakersfield (when permitted) in a flat top, cloth-covered, fiberboard casket.
FOR PLANS A, B & C, THE FOLLOWING SERVICES ARE ALSO INCLUDED: The following services are also included: Procurement and recording of vital statistics, certification of death with the appropriate Vital Records office, securing and payment of required permits and fees (except County Permit fee, Department of Consumer Affairs fee and sales tax) and counseling on Social Security, insurance or other benefits.
Removal and transport to the mortuary within the counties of San Luis Obispo, Santa Barbara and Ventura is included in the quoted price for McDermott-Crockett Mortuary.

Additional Options Available
The original certification of death stays with the County. Certified copies are available from the mortuary for an additional charge. The cost is determined by the County Recorder at the time of the request.
Ashes may be scattered at sea for an additional charge, or shipped anywhere in the USA for an additional charge.
While many members wish only the most simple care (usually with separately-arranged memorial services later), others may desire something more such as upgrading the casket, or providing casket-viewing. There are also times when circumstances require autopsy or embalming. To meet such needs, optional services are available at Society-contracted prices, to be added to the selected basic service plan. These arrangements are made by the member and the mortuary.

What is arranged and how payment is made is a matter between the member and the mortuary.
Questions or advice about details of any listed mortuary service should be directed to the Mortuary. For assistance on procedures or completing the required forms, call the Society at (805) 543-6133 or the Mortuary. Once you have filed your Mortuary Instructions they may be amended in writing, replaced, or withdrawn at any time.
